For students receiving services under IDEA, transition planning helps connect school experiences with life after high school, including education, employment, adult services, independent living, and community participation.
- Federal IDEA requirements call for transition planning no later than the first IEP in effect when the student turns 16.
- Transition goals are based on age appropriate transition assessments.
- Planning can address education, training, employment, and independent living when appropriate.
- Transition planning should be individualized rather than based only on a diagnosis.
