Understanding what the support actually involves.

Supported employment is a broad approach designed to help disabled people obtain and maintain paid work in integrated community settings. Services may include identifying job interests and strengths, job development, matching a person with appropriate work, interview preparation, workplace training, job coaching, accommodations, transportation planning, employer education, communication support, and continuing assistance after hire. Some programs provide intensive workplace based transition experiences before employment. Because supported employment includes many different models, evidence for one specific program should not automatically be assumed to apply to every vocational service.

The clearest evidence comes from Project SEARCH plus ASD Supports, which has produced large employment differences compared with usual transition services in randomized studies. Evidence also suggests that some participants maintain employment and may gain independence. However, research is concentrated in a relatively small number of programs and research teams, and much less is known about older autistic adults, long term career development, job satisfaction, wages, full time employment, quality of life, or which support components matter most. Successful employment should be judged by job fit, sustainability, autonomy, adequate support, and the person's own goals rather than employment status alone.

Structured workplace based supported employment can substantially increase competitive employment for some autistic young adults transitioning from school into adulthood.

Why did we rate it this way?

A multisite randomized clinical trial included 156 autistic participants ages 18 to 21 with significant support needs.
One year after graduation, 73.4 percent of the intervention group obtained competitive employment compared with 17 percent of controls for whom employment data were available.
An earlier randomized trial found 90 percent of participants receiving Project SEARCH plus ASD Supports obtained competitive employment three months after graduation compared with 6 percent of controls.
In that earlier trial, 87 percent of intervention participants remained employed at 12 months compared with 12 percent of controls.
A systematic review of randomized vocational interventions found evidence supporting Project SEARCH plus ASD Supports for autistic young adults.
Only a small number of autism specific randomized trials were available, and none were rated as having low overall risk of bias.

Employment status is measured much more often than job quality.
The multisite Project SEARCH trial reported an average of about 21 hours of work per week among employed intervention participants one year after graduation.
Research rarely follows autistic workers long enough to assess promotions, career progression, wage growth, or movement into preferred occupations.
A person can be technically employed while still being underemployed or working in a poorly matched job.
Future research should distinguish simply obtaining employment from obtaining sustainable, adequately compensated, personally meaningful work.

Most controlled employment studies prioritize whether a participant obtained a job rather than long term wellbeing.
Poor job fit, inaccessible environments, masking demands, discrimination, or inadequate accommodations can make employment harmful or unsustainable even when a person remains employed.
More research is needed using outcomes defined as meaningful by autistic workers themselves.

Who Was Actually Studied?

Applicability matters.

Age and life stage

The strongest controlled evidence involves transition age youth approximately 18 to 21 years old. Research involving adults across the broader working lifespan is substantially more heterogeneous.

Intellectual disability

People with developmental and intellectual disabilities are included in some employment research, but autism specific subgroup reporting is inconsistent. Some Project SEARCH studies involved participants with significant support needs.

High support / profound autism

Important controlled studies specifically included young people described as having significant impact from autism. However, eligibility requirements in some trials, such as independent self care, mean the evidence should not be assumed to represent every person with profound or very high support needs.

Minimal or no spoken language

Communication support needs are not consistently described in employment intervention studies. Minimally speaking and nonspeaking autistic adults remain poorly characterized as a separate population.

Adults

Adults are the target population for employment support, but the strongest experimental evidence is concentrated around the transition from high school. Evidence becomes thinner across middle adulthood and older adulthood.

Settings

Employment interventions occur in workplaces, community settings, schools, internship sites, vocational rehabilitation programs, and employer based programs. Workplace context and employer participation can strongly influence outcomes.

Autistic Perspectives

Research evidence and lived experience answer different questions.

Research centered on autistic workers shows that employment success depends on much more than individual job skills. Autistic adults describe workplace environments, social expectations, sensory conditions, communication differences, employer attitudes, accommodations, and acceptance as major influences on whether employment is sustainable. The evidence increasingly supports shifting some responsibility from requiring autistic workers to adapt toward designing workplaces that adapt to autistic workers.
