Parent mediated support and caregiver coaching are broad approaches in which a professional teaches a parent or caregiver strategies to use with an autistic child during everyday activities and interactions. Programs may focus on communication, play, social engagement, adaptive skills, emotional or behavioral support, or other goals. Examples include parent mediated naturalistic developmental behavioral interventions, Pivotal Response Treatment, Project ImPACT, parent mediated Early Start Denver Model approaches, and other coaching programs. Because these interventions differ substantially, evidence for one specific program should not automatically be assumed to apply to every form of parent training or caregiver coaching.

Project ImPACT significantly improved parent fidelity compared with treatment as usual and parent education conditions in the network meta analysis.
Parent mediated PRT and ESDM programs also showed strong improvements in parent implementation measures.

Who Was Actually Studied?

Age and life stage

The strongest evidence involves toddlers, preschoolers, and younger school age children. Evidence becomes substantially thinner for adolescents.

Intellectual disability

Children with intellectual and developmental disabilities are represented in some studies, but intellectual disability is not consistently analyzed separately and many programs have communication or learning requirements that affect accessibility.

High support / profound autism

Children with substantial support needs are represented in parts of the literature, but modern definitions of high support or profound autism are rarely reported consistently and subgroup results remain limited.

Minimal or no spoken language

Minimally speaking and nonspeaking children participate in some parent mediated communication studies, but communication profiles and AAC use are not consistently analyzed separately.

Adults

Parent mediated intervention research overwhelmingly concerns children. It should not be generalized to autistic adults, although family or caregiver education may still be relevant in some adult support contexts.

Settings

Programs are commonly implemented at home and in everyday routines, with coaching delivered in clinics, homes, community programs, groups, or through telehealth.
